{"id":124,"date":"2008-05-09T21:33:22","date_gmt":"2008-05-09T19:33:22","guid":{"rendered":"http:\/\/blog.ultimatefatburner.com\/wordpress\/acid-base-balance-myths-and-realities\/"},"modified":"2015-12-03T11:14:56","modified_gmt":"2015-12-03T16:14:56","slug":"acid-base-balance-myths-and-realities","status":"publish","type":"post","link":"https:\/\/www.ultimatefatburner.com\/ufb-blog\/acid-base-balance-myths-and-realities\/","title":{"rendered":"Acid-Base Balance: Myths and Realities"},"content":{"rendered":"<p>If you Google terms like &#8220;alkaline diet&#8221; or &#8220;alkalizing foods&#8221; &#8211; you&#8217;ll come up with page after page of some of the loopiest stuff imaginable.\u00a0 One site proclaims that an &#8220;acidic, anaerobic&#8221; body encourages the growth of &#8220;fungus, mold, bacteria and viruses.&#8221;\u00a0 Another provides a laundry list of non-specific symptoms of &#8220;acidification&#8221; and suggests that stimulants such as &#8220;coffee, tea and alcohol&#8221; are &#8220;extremely acidifying.&#8221;\u00a0 Often, sites will include lists of acid and alkaline foods&#8230;and if you compare lists, they\u00a0don&#8217;t always\u00a0match.\u00a0 And needless to state, no one tells you where this information comes from.\u00a0 Are the authors relying on scientific studies?\u00a0 Something they read in a holistic health magazine written by\u00a0a &#8220;nutritionist&#8221; with vague credentials and training?\u00a0 Pulling it out of their you-know-where&#8217;s?\u00a0\u00a0 Who knows?\u00a0\u00a0In addition, many of these sites\u00a0&#8211; not coincidentally &#8211; have\u00a0diverse &#8220;alkalizing&#8221; supplements and cure-alls to sell you&#8230;such as\u00a0<a target=\"_blank\" href=\"http:\/\/www.amazon.com\/Green-Kamut-Wheat-Grass-juice\/dp\/B000CL4JP4\" rel=\"noopener\">wheatgrass juice<\/a>,\u00a0<a target=\"_blank\" href=\"http:\/\/www.evenbetternow.com\/proddetail.asp?prod=AlkaLife\" rel=\"noopener\">potassium\/sodium hydroxide drops<\/a>,\u00a0<a target=\"_blank\" href=\"http:\/\/www.organickingdom.com\/capra-mineral-whey-4632.html\" rel=\"noopener\">goat whey<\/a>, cesium chloride\u00a0and\u00a0<a target=\"_blank\" href=\"http:\/\/kangen-water.ws:80\/\" rel=\"noopener\">alkaline water<\/a>.<\/p>\n<p>There is, however, a reality behind the hype.\u00a0 The subject of acid-base balance is one of increasing concern to researchers studying osteoporosis and sarcopenia (age-related muscle wasting).\u00a0 Both of these\u00a0conditions are associated with aging.\u00a0 Maintaining a proper acid-base balance may be the key to preventing both.<\/p>\n<p><!--more-->Before digging in deeper, it will help to have some background&#8230;what do we mean by acid-base balance?<\/p>\n<p>The measurement that defines acidity, pH, is a way of assessing the number of free H<sup>+<\/sup> molecules (hydrogen ions)\u00a0in a solution. pH is expressed as a number between 1 and 14: 7.0 is neutral,\u00a0 below 7.0 is acidic, and\u00a0greater than\u00a07.0 is basic (or alkaline). The pH of blood is slightly alkaline, around 7.4. The body requires a relatively constant pH to function properly. Changes in pH can affect the ionization of the molecules participating in important physiological reactions.\u00a0 Even relatively small changes would be sufficient to shut down many processes, and could result in coma, or even death. The pH of the blood must be maintained within a very narrow range, <u>+<\/u> 0.3, to function normally.<\/p>\n<p>In other words, &#8220;acid-base balance&#8221; is about maintaining your blood at an optimal pH &#8211; which is ever-so-slightly alkaline.<\/p>\n<p>So where does diet fit into all of this?<\/p>\n<p>Unfortunately, our Western-style diet presents a challenge to our bodies&#8217; ability to maintain itself at the correct pH level.\u00a0 Researchers have identified three culprits: cereals\/grains, animal proteins, and salt (sodium chloride).\u00a0 Diets dominated\u00a0by these foods\u00a0create a condition known as <strong>chronic metabolic acidosis<\/strong> (CMA). A more severe form of this syndrome occurs in kidney disease. As kidney function deteriorates, the ability of the body to eliminate metabolic wastes is affected. Some of these waste products are acids. As they build up in the bloodstream, the normal, slightly alkaline pH of the blood changes. The results are devastating. <a target=\"_blank\" href=\"http:\/\/www.smw.ch\/docs\/pdf200x\/2001\/09\/smw-09666.pdf\" rel=\"noopener\">According to one summary<\/a>:<\/p>\n<p><em>&#8220;Metabolic acidosis induces nitrogen wasting and, in humans, depresses protein metabolism. The acidosis-induced alterations in various endocrine systems include decreases in IGF-1 levels due to peripheral growth hormone insensitivity, a mild form of primary hypothyroidism and hyperglucorticoidism. Metabolic acidosis induces a negative calcium balance (resorption from bone) with hypercalciuria and a propensity to develop kidney stones. Metabolic acidosis also results in hypophophataemia due to renal phosphate wasting. Negative calcium balance and phosphate depletion combine to induce a metabolic bone disease that exhibits features of both osteoporosis and osteomalacia.&#8221;<\/em><\/p>\n<p>The effects of diet-induced CMA are\u00a0more insidious. The damage occurs in small increments, much like the effects of erosion. Over time, however, the cumulative effects become impossible to ignore. As noted above, low-grade CMA has been implicated in many of the symptoms associated with aging: <a target=\"_blank\" href=\"http:\/\/ajcn.nutrition.org\/content\/79\/1\/131.full\" rel=\"noopener\">loss of bone<\/a>, <a target=\"_blank\" href=\"http:\/\/press.endocrine.org\/action\/cookieAbsent\" rel=\"noopener\">muscle wasting<\/a>, <a target=\"_blank\" href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/15133327?ordinalpos=25&amp;itool=EntrezSystem2.PEntrez.Pubmed.Pubmed_ResultsPanel.Pubmed_RVDocSum\" rel=\"noopener\">formation of kidney stones<\/a>, <a target=\"_blank\" href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/9176376\" rel=\"noopener\">depression of thyroid hormones<\/a>, and an\u00a0<a target=\"_blank\" href=\"http:\/\/ajprenal.physiology.org\/content\/284\/1\/F32.full\" rel=\"noopener\">increase in cortisol secretion<\/a>.<\/p>\n<p>So how do cereals\/grains, animal proteins, and salt contribute to CMA?\u00a0 These foods release a variety of compounds during their breakdown, including sulfuric acid, derived from the metabolism of sulfur-containing amino acids in proteins, and phosphoric acid, released from animal protein foods, grains,\u00a0some nuts, and mature legumes (there are also some foods where phosphoric acid may be consumed directly, such as regular or diet colas). Chloride (Cl-) anions <a target=\"_blank\" href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/17522265?ordinalpos=1&amp;itool=EntrezSystem2.PEntrez.Pubmed.Pubmed_ResultsPanel.Pubmed_DiscoveryPanel.Pubmed_Discovery_RA&amp;linkpos=2&amp;log$=relatedarticles&amp;dbfrom=pubmed\" rel=\"noopener\">from table salt<\/a> <a target=\"_blank\" href=\"http:\/\/onlinelibrary.wiley.com\/doi\/10.1359\/jbmr.071118\/pdf\" rel=\"noopener\">contribute to acid levels as well<\/a>.\u00a0<\/p>\n<p>Blood contains a variety of compounds that are able to \u201cbuffer\u201d excess acid.\u00a0 The <a target=\"_blank\" href=\"http:\/\/scifun.chem.wisc.edu\/CHEMWEEK\/BioBuff\/BioBuffers.html\" rel=\"noopener\">primary buffer system in the blood is the bicarbonate buffer system<\/a>. Carbonic acid (H<sub>2<\/sub>CO<sub>3<\/sub>) acts as a hydrogen (proton) donor, and hydrogen carbonate (HCO<sub>3<\/sub><sup>&#8211;<\/sup>) acts as a hydrogen acceptor. The amounts of each component are regulated via two separate systems. Carbonic acid levels are regulated via respiration. Excess carbonic acid is converted to carbon dioxide gas by an enzyme, carbonic anhydrase, and exhaled from the lungs. Excess hydrogen carbonate is filtered through the kidneys and excreted in urine. Hydrogen carbonate is present in much higher quantities than is carbonic acid; this gives the body the ability to take up excess protons, and eliminate them in the form of carbon dioxide and water.<\/p>\n<p>If excess acid is produced, an increased amount of hydrogen carbonate is lost. If the carbonate is not replaced by the diet, it must be taken from the body\u2019s own stores. The main storage\u00a0depot for carbonate is the skeleton.<\/p>\n<p>In other words, our bones represent a sort of &#8220;safety valve,&#8221; against excess acidity.<\/p>\n<p>Thanks to our bones, our bodies have the means to counteract acidosis, but that protection comes at a price. Bone is composed primarily of calcium carbonate, calcium phosphate, collagen (structural protein) and water. Bone modeling and remodeling is conducted by specialized cells known as osteoblasts and osteoclasts. Osteoblasts are involved in bone formation, while osteoclasts are involved in bone resolving. Normally, a balance exists between the two activities: osteoclasts dissolve older bone, while the osteoblasts replace it. Bone weakening occurs when the rate of bone resorption by osteoclasts exceeds the rate of bone modeling by osteoblasts.<\/p>\n<p>Metabolic acidosis accelerates bone resorption by two mechanisms. Initially calcium losses occur through physicochemical dissolution of bone minerals. This is followed by <a target=\"_blank\" href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/11842949\" rel=\"noopener\">stimulation of osteoclast activity and depression of osteoblast activity<\/a>. <a target=\"_blank\" href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/12133196\" rel=\"noopener\">These\u00a0changes lead to pit formation in the bone<\/a>, and destruction of the organic matrix.\u00a0 Unchecked, these processes weaken the bones, and accelerate the onset of osteoporosis.<\/p>\n<p>The damage caused by CMA isn\u2019t limited to bone \u2013 muscle is also compromised.\u00a0 Muscle breakdown, or proteolysis, is a homeostatic response to acidosis.\u00a0 Muscle proteolysis releases the amino acid\u00a0glutamine, which\u00a0can then be used by the kidneys to produce ammonia. This conversion allows <a target=\"_blank\" href=\"http:\/\/press.endocrine.org\/action\/cookieAbsent\" rel=\"noopener\">increased excretion of protons in the form of ammonium<\/a>.\u00a0 <a target=\"_blank\" href=\"https:\/\/books.google.com\/books?id=ilgao9pjvQsC&amp;pg=PA279&amp;lpg=PA279&amp;dq=glutamate+and+protons+and+acid-base+balance&amp;source=web&amp;ots=NBPNKAbGe_&amp;sig=6d7VNGlufCiYmHCvDIBQiqdKKTo&amp;hl=en\" rel=\"noopener\">Glutamate is also used to consume surplus protons via conversion to glucose and urea<\/a>.\u00a0\u00a0<\/p>\n<p>Important hormones are also affected to some degree.\u00a0 The metabolic acidosis that accompanies kidney disease is known to cause increases in glucocorticoid hormones (cortisol, ACTH), decreased secretion of growth hormone (GH) and insulin-like growth factor-1 (IGF-1); decreased secretion of parathyroid hormone (PTH); decreases in free T3\/T4 thyroid hormones and an increase in thyroid stimulating hormone (TSH). These hormones all play a role in growth and body composition. The impact of milder, diet-induced acidosis on these hormones is less pronounced, but\u00a0still significant. Diet-induced acidosis is considered <a target=\"_blank\" href=\"http:\/\/ajprenal.physiology.org\/content\/ajprenal\/early\/2002\/09\/24\/ajprenal.00212.2002.full.pdf\" rel=\"noopener\">\u201c&#8230;part of an endocrine-metabolic continuum that includes\u2026moderate to severe CMA.\u201d<\/a><\/p>\n<p>The good news is that it\u2019s possible to correct CMA and arrest \u2013\u00a0or even reverse \u2013 the negative changes.\u00a0 Impressive results have been achieved with experimental bicarbonate supplementation.\u00a0<a target=\"_blank\" href=\"http:\/\/press.endocrine.org\/action\/cookieAbsent\" rel=\"noopener\">In one study<\/a>,\u00a0 60 \u2013 120 mmol\/day of orally administered potassium bicarbonate reduced endogenous acid production in post-menopausal women to zero. Furthermore, the researchers calculated that the decrease in nitrogen excretion during the experimental period \u2013\u00a0if\u00a0maintained\u00a0over a year \u2013 would result in a net gain of 2.0 kg (4.4 lbs.) of lean body mass &#8211; equivalent to \u201c\u2026a restoration of about 1 decade of muscle mass decline.\u201d <a target=\"_blank\" href=\"http:\/\/ajprenal.physiology.org\/content\/ajprenal\/early\/2002\/09\/24\/ajprenal.00212.2002.full.pdf\" rel=\"noopener\">In another study<\/a>, oral potassium\/sodium bicarbonate\u00a0supplementation dramatically increased calcium retention, while decreasing markers of bone resorption (deoxypyridinoline, pyridinoline, and n-telopeptide),\u00a0and plasma cortisol levels. This research was particularly striking in that the study subjects were young (22.1 + 1.4 years) adults still undergoing bone growth.\u00a0 In a more recent study, <a target=\"_blank\" href=\"http:\/\/jasn.asnjournals.org\/content\/17\/11\/3213.full\" rel=\"noopener\">potassium citrate was also shown to be\u00a0effective<\/a> for increasing bone mineral density and reducing calcium excretion (citrate is a metabolic precursor to bicarbonate).<\/p>\n<p>Could similar benefits be obtained through diet alone? Are there foods that can supply the body with a sufficient amount of alkaline salts to neutralize the effects of dietary acidosis?<\/p>\n<p>The answer just might be yes.<\/p>\n<p>Over the last decade, Thomas Remer and Friedrich Manz <a target=\"_blank\" href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/7797810?ordinalpos=4&amp;itool=EntrezSystem2.PEntrez.Pubmed.Pubmed_ResultsPanel.Pubmed_RVDocSum\" rel=\"noopener\">developed and validated a calculation model to estimate the renal net acid excretion (NAE) produced by various foods<\/a>.\u00a0 Their calculations were based on the principal non-bicarbonate anions (sulfate, phosphate, chloride, and organic acids) and cations (sodium, potassium, calcium, and magnesium) in foods\u00a0and validated using human subjects consuming different diets. Although their full list of 118 foods is far from comprehensive, it is sufficient to establish a pattern: fruits, vegetables, juices, and alkali-rich, phosphorus-poor beverages have the lowest (negative) scores. The scores increase towards neutral for alkali-poor, low-phosphorus beverages, fats and oils, and become positive for milk and non-cheese dairy products, and grain foods such as bread and noodles. Meat, poultry, fish, egg yolks and cheeses are the foods with the highest values.<\/p>\n<p>In other words, while the positive scoring, high protein and grain foods contribute acids, other foods \u2013 primarily vegetables, herbs, and fruits with negative scores \u2013\u00a0produce alkaline metabolites that can counteract them and prevent acidosis from developing.\u00a0<\/p>\n<p>Unfortunately, veggies and fruits are precisely the foods that are displaced in modern, Western diets.\u00a0 <a target=\"_blank\" href=\"http:\/\/ajph.aphapublications.org\/action\/cookieAbsent\" rel=\"noopener\">Surveys<\/a> consistently show that a majority of Americans don&#8217;t meet even the minimum dietary recommendations for vegetable\/fruit consumption.<\/p>\n<p>Can consuming more veggies and fruits prevent &#8211; or at least mitigate &#8211; the effects of CMA?\u00a0A number of studies suggest\u00a0it can. Several epidemiological surveys have been conducted that firmly link higher consumption of alkali-rich vegetables and fruits with\u00a0<a target=\"_blank\" href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/18326605?ordinalpos=1&amp;itool=EntrezSystem2.PEntrez.Pubmed.Pubmed_ResultsPanel.Pubmed_RVDocSum\" rel=\"noopener\">lean body mass<\/a> and\u00a0indices of bone health in <a target=\"_blank\" href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/11842948\" rel=\"noopener\">older (ages 69 \u2013 97)<\/a>\u00a0and <a target=\"_blank\" href=\"http:\/\/jn.nutrition.org\/content\/133\/10\/3130.full\" rel=\"noopener\">middle-aged adults<\/a>; <a target=\"_blank\" href=\"http:\/\/ajcn.nutrition.org\/content\/71\/1\/142.full\" rel=\"noopener\">pre- and perimenopausal women<\/a>, and even <a target=\"_blank\" href=\"http:\/\/ajcn.nutrition.org\/content\/79\/2\/311.full\" rel=\"noopener\">adolescent girls<\/a>.
